You don't have to enter Pre-order or Promotional codes to make an account. Those options are there for people who have them. You could only get them if you pre-ordered the game, or recieved them from some sort of promotional event for the game.

Actually, the whole time your are on the "newbie island" playing solo you dont get charged a fee, you only start getting charged when you get off the "newbie island" and into the multiplayer stuff.
