Depends on the game's theme. A few that I've come up with for MMORPG ideas I've had:
Deus Vult Online (historical PvP/territory control set in 12th century Outremer)
Stalingrad Online (MMOFPS set in Stalingrad 1942)
1066 Online (tri-faction PvP set in semi-historical, low-fantasy 11th century England)
Jolly Roger Online (sailing-focused faction PvP set in semi-historical, medium-fantasy 18th century Caribbean)
Cops and Robbers Online (realistic/historical crime game set in 1930s New York/Chicago)
Hellas Online (themebox set in high-fantasy classical Greece/Anatolia)
Manifest Destiny Online (sandpark quasi-survival game set in the late 19th century American "wild" west)
Final Frontier (PvP sandbox/builder set on a recently discovered habitable planet around Alpha Centauri in the 23rd century (yes, I know it would get confused for Final Fantasy. That's part of the reason I like it))
