Honestly, the "only" MMO that comes to mind for what you're asking for is FFXI.
Every MMO I've ever played will allow you to solo for most of if not all of the game until you reach max level and then require groups to do things and see raid content, etc.
FFXI was so brutal to the casual gamer, it never took off like I probably should of. You cannot solo same level mobs after like level 10 as most classes. The only class that I know of that can solo from 1-Max is a Beastmaster, but that is kind of because that is what they're designed to do. No other class in the game can solo. You have to use zone chat in towns or in the zone to find people to group with and level up, do any quest, basically do anything period. Unlike most MMO's where you fight similar level things, in FFXI you have to fight mobs level 7-14+ levels higher than you. So running to a xp spot requires you to rely on others for tele-ports, you'll need silent oils and vanishing prisms to make you invisible to the mobs you're running past. If you pull aggro you'll probably get one shot unless you're a tank class. It was the most "team/party" based MMO I played.
Thieves would use trick attacks to dump massive hate onto the tank to make his job easier, groups always had to coordinate to do proper skill chains to weaken the mob to specific elements so the mage could do more damage. It really was a one of a kind game. And again, the only game I can think of that "requires" a group to get by. I cannot fathom how long it would take to try and level purely solo on a fresh account.
Edit: Totally forgot to mention.. Death is a big deal, you die, you lose exp, you lose enough exp, you de-level.
One of the main reasons why MMO's exploded was due to the solo content. Its kind of a give and take situation and probably somewhat hard to balance to keep everyone happy.
A required party for questing? Like main story/side story questing? Neverwinter Online, after you reach lvl 60 and have to run through Campaign zones. You can try to solo it if you are new to the game starting at lvl 60 but death comes quick in those zones if you don't have uber gear. Even average geared lvl 70's have a hard time and rather prefer to group up in those areas.
Just about every MMO has quests that require team play. If you're asking for an MMO in which every quest is group only, or 90% is group only, good luck.

I in fact added a lot more to this than you. If you would of read his entire post, you would known that FFXIV is not the type of game he wants. It is a themepark mmo, very similar to wow. I write this as I'm currently playing my level 60 Dark Knight in FFXIV. I love the game, but it isn't what the OP was asking about. Jackass.

FFXIV after you hit the level cap. before you hit the level cap you can solo. but when you are getting tokens etc to buy high end gear you need teamwork in dungeons. you have to rush them in 30 minutes to 1 hour about 4 times a day and do that for 2 months and you can have a full set of high end gear.

Pantheon: Rise of the Fallen, will likely be the only full on grouping game coming in the near future. Designed to be in the same scope as Early EQ and Vanguard which were probably some of the most unforgiving games requiring largely group play.
When I played FFXIV I choose to group as much as I could. I was in groups from the first dungeon and continued to play like that. I think you are more interested in what other people are doing if you want to group you can easily group in FFXIV and do so every time you log in.
Perhaps you should have said I want a game where others are forced to group and not require teamwork because FFXIV does require teamwork. Forced grouping will not fly anymore.
For duo questing I'd recommend Wizard 101. It's a game where you can solo if you want, but the battle systems are done so that doing the quests duo makes much more sense. Max. group size is 4 people so that you can do the quests in groups of up to 4, but personally I think normal quests get too easy if the group has more than 2 members.
I don't know any good games for questing in larger groups, but for grinding things in group Dofus and Wakfu are awesome games. They don't have that many quests (or at least didn't have when I last played them which is quite a long ago), but are really good for relaxing group grinding.
In the modern era of mmos, FFXIV is the closest thing atm I would say.
But with that said it is almost pure solo outside a few dungeons until you get to endgame.
We are in the massive soloplayer online genre. Where everyone solos everything and grouping involves 3-4maning stuff. And raids = full party.
Oh and forgot to mention in ffxiv outside top tier raid... dungeons require no teamwork or communication. Simply push out dps and have someone who can heal and you win. 0 communication is needed for anything.

Best game I can think of that encouraged grouping was old FFXI, game was designed purely around group game play for 95+% of the content. Then again it is why the game capped at 550k subs at peak. If you wanted to progress solo you simply could not outside of 2 classes really. Endgame it forced you to form a guild to progress.
Long story short there are NO NEW MMOS that have a focus on group based leveling experiance. Everything is Solo player until endgame.
I'm having a great time in TSW. I was very surprised with how many opportunities there have been to group up and do dungeon missions. I've never cared too much about grinding for leet gear, but I actually want to do them so I can experience the story. And <gasp> people actually chat.
PS. The anniversary golem events have also been fun to fight alongside dozen of people

I think you are asking for MMO that "dont' have solo content". Which is very few.
Almost every mmorpg have dungeons, raids, or group pvp. Those require team works.
