I agree partly with Lineage II. Last couple of years more and more instanced dungeons have appeared in that game, making it more and more less an open world than what it used to be
An other for the list for good open worlds: Fallen Earth. There are a few instanced dungeons but they use some special technique that you don't even notice you're entering one...
I agree with this list minus LOTRO. You actually do quite a bit of loading in that game (from questing). It's phasing is rather jarring and poorly done(much like ESO's).
