OK, here are my answers:

Age of Conan / FFXIV, and LotrO when it comes to purely environment graphics
This might change when the following MMO's come out:

Hard to say, it varies from server to server. I'd say the early MMO's in general, and to me AoC was the best community experience I had recently, but that was because of the server I was on, which was a great and creative one.
On average LotrO has the friendliest community though from the recent MMO's.
10. Best Player vs. Player content
EVE Online (allround), DAoC (RvR part), GW (arena PvP)
11. Most immersive experience?
EQ for me
12. Biggest world?
I gathered all available info and did some tests, EVE is biggest all around, and SWG, AC and DF Online seem to contend for the land based worldsizes (see the link in my sig for the details)

I would like to see best character advancement added to the list as well. For me, that'd go to Mabinogi.
I can only comment on a few, since I played so many MMORPGs that very few stand out.

Forsaken World, since quests are easy to get to.
Best Fluff content ( housing, pets.. etc.)
Aside from WoW, AQ Worlds (the Flash MMORPG)
Best Player vs. Player content
Hands down, Aika Global with the mass war/raids. Those stuff are EPIC. Probably best endgame as well, since raids are VERY fun once you grow strong.

Age of Conan is probably the best, I would say Lord of the Rings Online if it wasn't for the animations.
2. Smoothest Controls
This one's kind of hard to just pick one. It's easier to pick the ones that don't have smooth controls. You either got it or you don't.
3. Best User Interface
Easiest to use is probably World of Warcraft, it's pretty self explainatory, and even customizing it is simple.
4. Best Character creation
Champions Online is one of my favorites. You can create your own race if you want to.
5. Best Class and Race choices
Dark Age of Camelot had numerous classes, each unique to each faction. The races were also unique. I still haven't found a AAA title that has more class/race combinations.
6. Best PVE leveling content.
Final Fantasy 11 is probably my favorite. Almost the entire game requires you to group, and all the quests in the game actually mean something when you complete them. None of that "Go kill 20 Boars, because I said so" quests.
7. Best Crafting
Some hated it, but I really liked crafting in Dark Age of Camelot. As you got further into the craft you've chosen it takes longer and longer to make items.
For gathering materials I love how Age of Conan has it done. You somtimes get attacked by bandits and other miners while gathering from a node. I hate how long, how much money, or how many resources it takes to level up the actually crafting skills though.

Final Fantasy 11. I can't even count the amount of Gil and time I've spent on my Mog House...I know it's alot though.
9. Best community
There's a few games that come to mind when I say Community. The best would have to be Final Fantasy 11. The forced grouping makes everyone have to work together. All the ninjas and 12 year old crybabies usually leave by the time they get to level 20 in thier first job
10. Best Player vs. Player content
If anyone tells you anything else they're outright lying or they haven't played it. Dark Age of Camelot is the best PvP ever, and no other game comes even remotely close to it. PERIOD!
11. Most immersive experience?
The Roleplaying experience in Lord of the Rings Online is probably the most immersive I've ever seen. I could spend hours just chatting/running around Bree.
12. Biggest world?
Final Fantasy 11 has a huge world. Without a chocobo, it takes forever to get to Sandy from Windy. and that's just that one continent.

Dark Age of Camelot's RvR is just so good I could play it (and still do sometimes) 24/7/365
