How do expansions work in an MMORPG?

I understand how content updates work. You log on one day and the game is different. But what about expansions that you have to buy? If an expansion introduces a new zone, are you forbidden for entering that zone until you buy the expansion? Are you forbidden from getting items and quests that were introduced in the expansion? If not, then do people just buy expansions to raise their level cap?

Somebody, somewhere has better skills as you have, more experience as you have, is smarter than you, has more friends as you do and can stay online longer. Just pray he's not out to get you.
