Need a more clear path to blog creation

Related to site functionality, I'm having serious problems figuring out the UI related to blog creation.

 

A help desk thread gave me a direct link to a blog creation form.  The form didn't throw an error, but doesn't seem to have created a blank blog where I can post.

 

Any tips?  Is this a known issue?

    We're working on a site redesign at the moment and I'm sure we'll be cleaning up the blog area as part of it. In the interim, Grunty's instructions should be helpful!
