Types of Quests/Gear?

Just curious, are there any quests in this game where you go into a dungeon/castle/lair, etc., kill the boss, and get uber sword/armor, etc?

Also, how does one acquire most of their gear in this game?

There is a quest chain that gives pretty decent armor and I believe there is some raiding for better gear.

I just buy it from the auction house and it works fine. Then again, I mostly play this game by exploring and seeing what trouble I can get into so I don't really do quests unless they seem interesting. There are a lot of dungeons/keeps/temples in this game.
