Have a look at EVE Online; it's got open PvP with consequences of death harsh enough to give you that rush you can only really experience in UO and Darkfall. The problem is that the point-and-click nature of the game can make you feel a little detached from your character.
Frankly Darkfall is as good as it gets unless you want to move away from MMO's and onto the greatest PvP game ever made: QUAKE (www.nquake.com)
I did try out eve, and I must say while I like some of the concepts Im not sure I'd enjoy the game. I didnt like the trial, i'm more of a medival era fan.
Any MMO besides the one mentioned that you want probably aren't out yet.
The only games I can think of are Mortal and Dawntide.
Mortal is a game similar to Oblivion, but online and where you have to either craft everything yourself or buy it off another player, or at least, that's the way I see playing it. However, it's only core features so far and it's still in beta that only comes with pre-order, and is pretty broken in certain aspects. Combat seems fun, if not a bit slower paced to make it a little more tactical, but I haven't played with mobs active yet. All aspects of crafting, similar to Runescape, are a huge grind. I wouldn't recommend it to Runescape fans, but to the more mature Runescape vets looking for something a little more, it could be worth a shot. I'd also recommend it to UO vets, like yourself, as plenty have praised it for similarities to pre-Trammel UO.
Dawntide is still in pre-beta. The only way in is to get invited by a developer or current member of the beta, but it isn't that hard to get invited if you don't beg. It has many more similarities to UO than Mortal and promises a much more engaging combat system than the typical MMO setup. It's not a game for now, but it seems to be a game for the future, looks very promising and once it is more than 5% feature complete, could be a totally engaging experience.
Other than that, all I can think of is Sociolotron. The only reason I mention this is that despite all its debauchery, the game is solely about player freedom, roleplay and good ol PvP. Not to mention, the heir system is pretty neat. I've never played the game myself, but I've heard good things from those that do. Despite the debauchery, I also hear it's easy to avoid the debauchery with certain precautions. The only thing I'd have to say about it is that it's completely debaucherous and the graphics are ugly.
I agree with genius sage, once you play a game with risk AND pvp you'll never go back, everything else seems so meaningless. Give Eve online a good 2-3 months, I bet you'll enjoy it.. it won 2009 game of the year on this site and for good reason in my opinion.
